National Girls and Women in Sports Day at Lake Forest College!

The event is open to girls ages 8-14 and space is limited to the first 125 registrants.
All activities are FREE of charge.

Registration check-in starts at 12:30 p.m.
Program begins at 1 p.m.
(Participants should plan on eating prior to attending the event.)

Pick up begins at 3:15 p.m.*
*Participants have the option to attend the Forester women's basketball game and are allowed one adult guardian to join them free of cost.

**Please note that the event concludes at 3:15 p.m. Parent/Guardian/Group Leader(s) must be present for participant to stay at the Forester sporting events.**

Lake Forest College Athletics Image & Likeness Release Form

I hereby voluntarily grant Lake Forest College (the College) permission to use my likeness in photographs, video recordings or electronic images in any and all of its publications, including website entries, without payment compensation or any other consideration. I understand and agree that these materials will become the property of the organization and will not be returned. I hereby irrevocably authorize the organization to edit, alter, copy, exhibit, publish or distribute these images for purposes of publicizing the organization's programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image. I hereby hold harmless and release and forever discharge the College and its affiliates, trustees, directors, officers, partners, principals, employees and agents from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. I am 18 years of age and am competent to contract in my own name, or if I am under age 18, a parent or guardian has signed below. I have read this release before signing below and I fully understand the contents, meaning and impact of this release.

Medical Release

I voluntarily give permission for my child to participate in the Lake Forest College National Girls & Women In Sports Day (NGWSD) event and am aware that this agreement includes an assumption of risk. I release and waive Lake Forest College and its employees from any and all liability of any kind, especially in case of accident during activities related to NGWSD, as long as normal safety procedures have been taken. I hereby authorize designated staff of the College to act for me in any medical emergency and will assume all cost for any medical treatment required.
